Groupon Information

Please read over all information before calling:

1.Everyone will be required to fill out a full health history form located under New Patient Information. Please do not leave out any medication that you are currently on. Everything on your paperwork has reasoning behind it, so please do not leave anything out.

2.Everyone will be required to have a consultation. The consultation is FREE and it is for us to give all of the information you need to know about Laser Hair Removal before getting started. The consulation is still required even if you have undergone treatments at a different facility.

3. Everyone MUST bring in a hard copy of their Groupon. We do not have the technology to be able to scan mobile devices. This hard copy will be placed in your chart and will be shown as your payment.

4.The Laser Hair Removal package is for 5 treatments. All treatments must be used for same treatment area. Sharing packages with other individuals is not allowed.

5.Once you have used your package of 5 treatments, if you are wanting additional treatments, there is a maintenance fee applied. The cost for maintenance treatments will vary based on what part of the body will be treated. The price will be 1/2 of the individual price. 

6. Please review our office hours to make sure your schedule allows you time for your office visit.
      Monday: 9:00 am- 5:00 pm

      Tuesday: 9:00 am-5:00 pm

      Wednesday: 9:00 am- 5:00 pm

      Thursday: 1:00 pm- 8:00 pm

      Friday: 9:00 am- 5:00 pm

8. Areas may be upgradable to larger areas and can be discussed

 AT THE TIME OF CONSULTATION.

9. Even though we love children, please do not bring any with

 you! Children are not allowed in any of our treatment rooms, no exceptions!

10. PLEASE BE ON TIME. Try to get here atleast 10 minutes before your appointment time to make sure all of your paperwork is complete. You will be resceduled if you are not on time. We are trying to put as many people into our schedule as we can, so please be considerate with our time as we will be with yours!

11. If it is necessary for you to reschedule your appointment, we ask for a 24 hour notice. Due to the expected high volume of new clients and current client, it might take several weeks to months to get you back in, so please make this a priority.

12. If you are wanting to purchase full legs, full back, or chest AND abdomen you will need to purchase 2 "Large" Groupons.
